New Patient Forms
We would like to welcome you as a new patient of Dr. Kong in Acupuncture & Chinese Medical Center. We look forward to working with you to address all of your health concerns, and we will do all we can to ensure you achieve the best possible results that you can. The confidence and trust you have placed in us is greatly appreciated.
At your first visit, you will need to fill out the forms before your treatment. Please either arrives 10 minutes early before your appointment time to complete these forms, or print and complete them in advance and bring them to the office so that you will have more time to talk to Dr. Kong during your consultation.
As an option, you can bring a list of any supplements/ medications you have been taking, or/and copies of any relevant laboratory results or doctor’s notes.
